Wisconsin State Approved 120 hour program

COURSE FEE $750 includes books and materials 

Registration is limited to a maximum of 8-16 students, depending on location of course.

To register you must complete all of the following:

• Application form
• Self-assessment health form
• Background disclosure form
• Release for for background disclosure
• TB test form required before clinical
• Date of birth verification, such as a drivers license or birth certificate
• Social security Card to verify social security number for background check
• Payment for course. Fee is $750.00.
• Includes book and background check for Wisconsin (out of state requests must be paid by student).

Fee does not include registration for the Wisconsin Certified Nurses Assistant Exam

                  ($115 paid to Pearson Vue.)

CNA Registry Exam scheduling procedure materials distributed before end of class.

Chapter reserves the right to cancel an offering.

Students registered and paid will have the option to transfer to

another offering or a refund of their course fees.
